Assessment, planning, and
support for your family
As an Aging Life Care Professional, I offer an array of services to support older adults and their family caregivers.
Step 1: A comprehensive assessment
I start with an assessment of the older adult and their current status and living situation. The areas I assess include:
Health and Disability: Understanding the nature of an older adult’s limitations and diagnoses and the care they are currently receiving.
Goals of the older adult and family caregivers: Is the goal to keep a loved one in their home or to arrange for the best care in a different location? Does the older adult and all family members share the same goals?
Support Systems: Determining who are the important individuals for the continued support of the older adult. Is there a specific family caregiver or more than one? What are their needs and concerns?
Living Environment: Assessing the home environment to identify any needs for ensuring someone can remain in their home safely.
Legal Issues: Understanding the status of key estate documents such as Medical Power of Attorney and Durable Power of Attorney.
Finances: What are the financial resources available for providing care and what resources may be needed.
Step 2: Creating a Collaborative Plan
After identifying a client’s status, goals and needs, I can then put together a plan that includes problem-solving and prioritizing those needs with a longer-term view to the future. I meet with the family, including virtual meetings to include family members who live elsewhere, and facilitate the discussion around the care plan to answer questions and concerns and land on a plan everyone can support. The plan can include:
Finding, coordinating and managing in-home care.
Referral to financial and legal resources.
Recommendations for home modifications.
On-going coaching for family caregivers.
Attending doctor appointments and facilitating communication between the doctor, client and family caregivers.
Assisting in paying bills and managing finances.
Support & Crisis Intervention
Particularly when there isn’t a family caregiver nearby, I can serve as the primary point of contact and be available for crisis intervention to help clients navigate through emergency departments and hospitalizations, rehabilitation stays and being their advocate to ensure that they are receiving the best care possible.
